Introduction

This is a demo of the feedback form script generated by thesitewizard.com's Feedback Form Script Wizard. The wizard allows you to generate a customized contact form for your site.

Feel free to test this form by sending a message (even "asdf asdf"). Note though that the message will be automatically discarded so don't write any real feedback here. No one will read it.
For this demo, all fields are required, although you can enter false information into those fields if you wish. For your real form, you may make any or all of these fields optional, if that's what you prefer.
The feedback form can look as pretty or as ugly as you want it to look. It's up to you to design your page to suit your tastes. The Wizard does not force the page to have any specific design. The appearance of this page just happens to be one I created for the demo. It's deliberately made to look different from the rest of thesitewizard.com so that you don't mistake the demo for the real thing.
If you need additional spam protection for your forms, you can also get the Wizard to add a CAPTCHA test to your form.
